When most people say “botox,” they’re really talking about neuromodulators, which includes the most household name BOTOX® Cosmetic, but also includes other neuromodulator brands like Dysport® and Xeomin®. Regardless of what you call them, these are purified proteins that temporarily relax targeted muscles, softening wrinkles caused by repeated expressions (frowning, laughing, raising your eyebrows, etc.). For the sake of simplicity, I’ll be referring to neuromodulators in general as botox throughout this guide.

Well, I am one of the very rare people who is immune to all neuromodulators. While I don’t have an exact percentage for you, I know this immunity is quite rare.

How does botox work?
Botox is a purified form of botulinum toxin that blocks the signal between nerve and muscle. When injected into specific muscles, it prevents them from contracting, which stops the repetitive creasing of the skin that leads to fine lines and wrinkles. Beyond wrinkle reduction, botox can also help with things like teeth grinding (bruxism) and excessive sweating (hyperhidrosis). But in my practice, it’s most often used to treat dynamic wrinkles (the lines caused by facial movement) like frown lines, crow’s feet, and forehead lines. And yes, men get these too. If you’ve noticed them and feel a bit self-conscious, you’re not alone. Men may not talk about their appearance as much, but that doesn’t mean we don’t care.
As a male cosmetic injector myself, I get it. We may want the treatment, but don’t feel like advertising it to our buddies. My approach to cosmetic injections focuses on subtlety, always working with your natural anatomy to keep you looking like yourself, just a bit less tired.

How is botox different for men?
The biggest difference between botox for men and women comes down to dosage and anatomy. Because men’s facial muscles—especially in the forehead, brow, and glabella (between the eyebrows)—are stronger and bulkier, they typically need 1.5 to 3 times more units than women to achieve the same level of relaxation. This strength creates deeper lines over time and means treatment has to account for both muscle mass and movement patterns. Men also tend to have thicker skin, which influences how botox diffuses and how visible dynamic wrinkles appear compared to women with thinner skin. On top of that, male brows are naturally straighter and lower, so preserving this shape is key to maintaining a masculine look and avoiding the overly arched appearance often desired by women. Some men may also metabolize botox faster or use their facial muscles more forcefully, which can slightly shorten how long results last.
How many units of botox do men need?

Most treatments take less than 20 minutes, and the injections themselves feel like a small pinch—often compared to a bug bite—with very minimal discomfort. Some mild redness or swelling may appear right after, but it usually fades within hours. You’ll start to see changes in a few days, with full results in about one to two weeks. Aftercare for men’s botox is simple but important. Avoid heavy exercise, saunas, or hot tubs for the first 24 hours, stay upright for a few hours after your appointment, and skip alcohol or blood thinners around treatment to minimize bruising. Following these steps helps your results settle smoothly and last as long as possible.

Will my wrinkles get worse if I stop botox?
To put it simply, no. There’s no such thing as a “rebound wrinkle.” When you stop getting botox, your muscles do not go into overdrive. For the period of time that your muscles were temporarily blocked from contracting, no new wrinkles form. And when you stop, your wrinkles simply return to as they were. Even if you only get botox a couple times a year, you will have slowed down the aging process by temporarily stopping dynamic wrinkles that form into static wrinkles. There’s a great twin study that I encourage you to look up as it shows the preventive measures of botox beautifully.

Is botox safe for men?
Safety is always my top priority, whether we’re talking about botox, dermal fillers, or other cosmetic injections. Botox has been Health Canada approved for both medical and cosmetic use for over 20 years, with a strong track record of effectiveness and safety for both men and women. That said, your results and safety depend heavily on the skill of the injector you choose.
In British Columbia, neuromodulator injections like botox can be administered by Naturopathic Doctors (that’s me!), physicians, nurse practitioners, and dentists. Registered nurses and licensed practical nurses may also perform injections, but only under the delegation and supervision of a licensed prescriber, since botox is a prescription medication that must be ordered by an authorized healthcare provider.
Ensure you’re booking with a trained, qualified professional who understands male facial anatomy—and watch for red flags. This not only maximizes safety but also ensures you get the results you’re after.
Does botox hurt?
Most describe botox injections as a quick pinch—like a bug bite—that’s over in seconds. The crow’s feet area can be a bit more sensitive, but discomfort is still minimal.
As a Naturopathic Doctor and experienced injector, I use techniques like numbing cream, ice, or cannula injections to make treatments as comfortable as possible. I encourage patients to let me know about any fears, anxieties, or history of fainting to allow me to adjust the appointment and give you extra time. My focus is on preserving natural movement while softening lines, so you’ll still look like yourself, just more rested. For men, this means paying close attention to male facial anatomy, such as keeping the brow flatter and avoiding an overly arched look. The right dosage and placement ensure you keep your masculine features intact and injections undetectable.
What are the risks or side effects of botox?
Botox is considered very safe when administered by a qualified professional, but like any medical procedure, there can be side effects. The most common are mild and temporary, such as redness, swelling, or bruising at the injection site. Occasionally, patients may experience a mild headache or tenderness in the area. Rarely, side effects can include eyelid or brow drooping, which typically resolves on its own in a few weeks.
